Job Summary: The Clinical Case Manager, Private Pay will coordinate and oversee comprehensive care plans for clients receiving Nursing, PCHM, or HHA services. Collaborating with the Care Center Team, they will assess client needs, develop personalized care plans, and ensure that our Caregivers are delivering quality care. They will establish regular updates with Primary Nurses and determine when in-person visits are necessary. The Clinical Case Manager may attend discharge planning and family meetings to facilitate home discharge and will be involved in intake to ensure appropriate care levels through discussions with staff and family. They will work with the Care Center to ensure HHAs meet skill requirements, address training needs with the Care Center and Education and Training, and help orient PC/HM and HCAs
Essential Duties and Responsibilities:
Client Assessment and Care Planning:
Conduct initial and ongoing comprehensive assessments with the Primary Nurse.
Develop and implement individualized care plans.
Adjust care plans based on health status changes.
Care Coordination and Management:
Coordinate care among healthcare providers for seamless service delivery.
Supervise and support home care staff.
Ensure compliance with regulatory and organizational standards.
Client and Family Communication:
Serve as the secondary contact for clients and families.
Document regular health updates and progress.
Address concerns and questions from clients and families promptly.
Quality Assurance:
Monitor care quality through regular follow-ups and reviews.
Conduct home visits to evaluate caregiver performance and client satisfaction.
Implement quality improvement initiatives based on feedback.
Documentation and Reporting:
Maintain accurate records of assessments, care plans, and progress notes.
Prepare and submit required reports.
Ensure documentation complies with legal and organizational standards.
Education and Training:
Participate in training and ongoing education for home care staff on best practices.
Stay updated on home care developments and integrate new knowledge into practice.

Bachelor of Science in Nursing (BSN) required; master's degree in nursing or healthcare administration preferred.
Current Registered Nurse (RN) license in the state of MA
Minimum of 5 years of experience in home care, case management, or a related field.
Previous experience in a supervisory or managerial role preferred.
Strong clinical assessment and care planning skills.
Excellent communication and interpersonal skills.
Ability to work independently and manage multiple priorities.
Proficiency in electronic health records (EHR) and care management software.
Valid driver’s license and reliable transportation for home visits.
Work Environment and Physical Demands:
This position involves a combination of office work and field visits to clients’ homes.
Prolonged periods of walking, bending and climbing to care for clients.
Must be able to stand for up to 30 minutes at a time.
Must be able to lift 50 lbs. on a regular basis.
The role requires travel within the service area of MA 50%

Pay Range:The hiring range for this position is $70,000-75,000/annually. Various factors will determine final compensation such as a candidate’s years of relevant work experience, skills, certifications, and location.
